Retatrutide (LY3437943) is an investigational triple agonist — a single peptide that activates three receptors: GIP, GLP-1, and glucagon. The added glucagon arm is what distinguishes it from dual agonists like tirzepatide, and it is driving some of the largest weight and liver-fat reductions reported in early trials.
How retatrutide works
Retatrutide engages three metabolic receptors at once. The GLP-1 and GIP arms curb appetite and improve insulin and glucose handling, while the glucagon-receptor arm adds increased energy expenditure and mobilisation of liver fat. Combining all three in one molecule is the mechanism behind its outsized effects in Phase 2 research.
What the research shows
- Obesity, Phase 2 (Jastreboff et al., NEJM 2023). In 338 adults with obesity, the 12-mg dose produced ~24.2% mean body-weight reduction at 48 weeks versus ~2.1% on placebo.
- Type 2 diabetes, Phase 2 (Rosenstock et al., Lancet 2023). In adults with type 2 diabetes, retatrutide gave dose-dependent HbA1c improvements and up to ~16.9% weight loss at 24 weeks.
- Liver fat / MASLD, Phase 2a (Sanyal et al., Nature Medicine 2024). At 8–12 mg, hepatic fat fell ~81–82% at 24 weeks, with most participants reaching normal liver fat.
Important candour: retatrutide is investigational — its data come from Phase 2 trials, with larger Phase 3 studies ongoing, so long-term efficacy and safety are not yet established. Use belongs under a physician’s guidance.
