GHRP-2 (pralmorelin) is a synthetic hexapeptide growth hormone secretagogue and the first of its class introduced clinically — used in Japan as a diagnostic GH-stimulation test. Like other GHRPs, it acts on the ghrelin receptor to trigger a strong GH pulse.
How GHRP-2 works
GHRP-2 is an agonist at the GHS-R1a (ghrelin) receptor on pituitary somatotrophs, producing potent, pulsatile GH release. Because healthy people respond with a marked GH rise while GH-deficient patients stay blunted, it is used as a single-injection diagnostic test. It also stimulates appetite and produces modest rises in ACTH, cortisol and prolactin.
What the research shows
- Diagnostic validation (Chihara et al., 2007). Validated single-injection GHRP-2 as a diagnostic test for adult GH deficiency.
- Adrenal-axis signal (Yuasa et al., 2016). A GHRP-2 peak-cortisol cutoff predicted secondary adrenal failure with about 90% sensitivity/specificity.
- Detection method (Okano et al., 2010). An LC-MS/MS method for pralmorelin (GHRP-2) and its metabolite in human urine.
