Cagrilintide (AM833) is an investigational long-acting amylin analog — a peptide engineered for once-weekly dosing that works through the body’s amylin and calcitonin receptors to promote satiety. It is most often studied alongside semaglutide as the combination known as “CagriSema.”
How cagrilintide works
Amylin is a hormone co-secreted with insulin that signals fullness. Cagrilintide is an acylated, long-acting version that agonises amylin and calcitonin receptors, promoting satiety, reducing food intake, and slowing gastric emptying. Because it works through a different pathway than GLP-1, researchers pair it with semaglutide to combine the amylin and incretin routes to appetite control.
What the research shows
- Weight, Phase 2 (Lau et al., Lancet 2021). In 706 adults with overweight/obesity, once-weekly cagrilintide (0.3–4.5 mg) produced dose-dependent weight loss versus placebo, mainly with gastrointestinal side effects.
- CagriSema concept, Phase 1b (Enebo et al., Lancet 2021). Co-administering cagrilintide with semaglutide 2.4 mg was well tolerated, with a profile supporting once-weekly combined dosing and greater weight reduction.
- CagriSema in T2D, Phase 2 (Frías et al., Lancet 2023). In type 2 diabetes, cagrilintide plus semaglutide improved weight loss and glycaemic control versus semaglutide alone.
Important candour: cagrilintide is investigational — evidence is early-to-mid stage (Phase 1b/2), with Phase 3 studies ongoing, so it is not an established treatment. Use belongs under a physician’s guidance.
